My impression is it's the same all over.
You would think a tiny fundamentalist Christian predominant city like Roanoke would suppress gay stuff but it doesn't. There are several gay bars, gay beuaty pageants etc, and quite a bit of a gay scene when you consider it's small size and conservatism. There are many very large cities in the Midwest most of which I haven't visited. Chicago is a Midwest city, and quite open loveually, at least as much as NY and LA. I can't comment beyond that, as far as the midwest, because I am not well traveled in that part of the world.
