Vomiting in public also can not always be helped, and is also unhygienic. Being naked in public is becoming more and more acceptable, look at the beaches for instance, and is also only looked down upon due to primitive illogical taboos. Being dirty in public is offensive on the olfactory level and unhygienic, nothing to do with primitive illogical fears.
