3 - No, you are not bound to live in the state you put unless you are applying for a STNI visa. That question is more for statistical purposes.
4 - Again, no. Another statistical question and also for DIMA to be able to see that you will be able to support yourself when you arrive. Don't worry about being spot on with your answer to this one. Do, however remember to include the value of anything you will be bringing. For example any furniture, jewellery, etc. Not just cash.
