Let's try to make it more simple. I hope I would do it as simple as possible *lol*
Put all the documents you have exactly in the order how they say in the checklist. This will say ... On the top will be your form 47sk, after that certified copy of pbuttport, skills buttessment... etc........ and on the bottom will be the evidence of any change of name (if you have any changes).
So, when all the documents are sorted in a pile, start numbering from the bottom. Write with the pencil on the right up corner number 1 on the last document of the pile, then next will be 2... etc... go on until form 47sk. So the first page of the form 47sk will be for example page number 148 or so!

the sub-dividing tabs-flags you put exactly on the place where they asked. it will help case officer to find faster all the info needed.
You need to put it on the first page of the form 47sk, skills buttessment and IELTS certificate (if you have one)
