I stumbled during our application over the same questions as you are wondering about now... Not quite sure what the exact wording was, but on the one about the visa if you want to withdraw it in case its not approved I think I ticked No (no withdrawl) because we would want to see why it was declined. (you had no right on insight or on your money if you withdrew the application, on the other hand - no nomination, no employer right??). For the period of stay we said 4 years, thought we can always change things afterwards. The question on visas before we answered yes but left details blank. Or isnt there somewhere you can actually wright "holiday visa"? Basically I think this question only concerns long term visas, not travel. You definetly have to copy all pbuttports on any page that has wrighting or stamps on. And then have every page stamped as certified. If you kids are at school, dont they need some kind of school documentation as to which clbutt theyre in?? Put July 15th for the date you need the visa, although even that would be pretty quick. We lodged ours on April 18th, meds are now finalised (took two weeks because I had to have the full exam) but we're still waiting for the visa. If you can, go to a doctor that sends the results online, thats the quickest. And it might be good to check for an appointment for in about 3 weeks time. Thats when they requested ours. But be sure you know exactly which part of the exam each of you need (ie xray, blodd, full body exam...) before you actually go! You will need a photo each (or maybe even two) for the medical form, as they use it as a kind of identification by the doctor. They stamp the picture as genuine there.
