I didn't number the pages in my pros marriage visa but I did put them in a certain order. From memory, Mine and my daughter pbuttports, a covering letter listing what was included in the application, my application form, birth-marriage-divorce certs, tenancy agreement, police letter, stat decs and relationship statement. Then partner's in same order. Relatives-Friends stat decs re relationship, with print outs of messenger chats, phone bills, cards, letters, print out of photos etc at the bottom. Then attached it all together with a big bulldog clip. They scan all the documents which is why they don't want staples I think.
Yes, there are forms you have to print out and take along to the doctor DIMIA site but I'm sure it's there somewhere. Be darned if I can find it though. The Panel Doctor and Xray receptionist told me the Form number that I needed when I rang to make an appointment, and yes, they know where to send the paperwork to.
I know of someone who's had a telephone interview. I don't believe it's that common unless your application is not straight forward.
