When we had original appointment for our medical they asked for written report from his diabetic doctor that carries out his bi-annual check up (copy of the letter they send to his GP), we brought two check-up letters addressed to his GP from the hospital specialist, however his last appt was late 2005 - at the medical the doctor said they might request more up to date letter-info so we asked for a copy as soon as carried out (24-05-06 after visa medical) and we have copy.
requested: 108 Endocrinologist's report (based upon a current buttessment) required regarding diabetes, and hypertension, please. The report is to address history (including any previous hospitalisation), physical examination findings, state of target organs, management needs and prognosis.
Please include results of the following: resting ECG, glycosylated haemoglobin level, serum creatinine level, Urinary albumin-creatinine ratio (with appropriate predicted values), or 24-hour collection for urinary protein and creatinine clearance.
Our Panel Doctor is working flat out and directly with OH's specialist to get the information for him.
