I can tell you what happened for ours. It might help in answering your questions.
Firstly, we were not asked by the Australian authorities to get medicals, but I'm trying to get employer nominated, and if I get my RPL done by the Australian Computer Society, we need to get to the next stage in the visa process quickly.
For us, the medical was in 2 stages. First stage was giving blood, and having check xrays. Both had supporting hospital forms, that they stapled a pbuttport photo to, and signed across it to verify it . We were given the forms and xrays in a sealed envelope which was stamped by the hosptial to show it had not been tampered with.
The results of the blood tests were then posted to one of the nominated docotors approved by the Australian authorities (listed on the immigration website. (they were sent to wrong doctor, but that is another story).
About 10 days later, we had appointments with the doctor, where we filled out one part of the form, and then he filled out the main parts, based on some questions, and a very basic examination. Strip to my undies, ran a finger down my spine (not sure why), tickled me (reflexes), took blood pressure. Everything else was questions, so no drop and cough, or finger up the bum despite asking for it! He noted I have tattoos (this means hepibreastus is needed to be included in the blood test, and excuse spelling), and a load of 4-10cm scars (I mean a lot) from having some lipoma (fatty lumps) removed.
He asked my GF about her glbuttes, and what strength they were, she didn't know, so he said 'easier not to mention them'!
Forms were completed, by the doc, and he also checked we had done our bits completly. Both of us had missed a section, which he picked up on, and said it was a common mistake. More pbuttport photo's attached, and signed.
The cost of the medical would have included postage to send everything to whereever a visa would be processed. But because we were not requested for medicals, there was no address to send then on to.
We had to wait to get the blood test results faxed, and then the whole lot was sealed in envelopes, and stamped, and then sealed in the xray envelopes, and stamped, and given to us. At some time, we will have to post them to the immigration centre in Brisbane, which I think is specific to ENS migration.
