I hope there is someone able to help me in making a statutory declaration. My problem is that, my birth certificate and pbuttport show only my first name Johanes on them, meanwhile my academic certificate and transcript show my full name Johanes Rudi Siswanto on them. As per ACS advise, a self-statutory-declaration should be sufficient to clarify the discrepancies.
So, I found few templates for general statutory declaration on the web, and made a draft for my own as copied below. Would really really appreciate if anyone could give me inputs for necessary corrections-changes. But, since I live in Indonesia, I am not really familiar with the common OZ practice, and have difficulties with the wordings :-p Again, would really appreciate if you could also advise me on these matters:
1. Is a witness a mandatory for my statutory declaration? Can I pick anyone to be the witness? 2. Is everything mentioned in my statutory declaration sufficient to clarify the name mismatch?

STATUORY DECLARATION
I Johanes Rudi Siswanto of my permanent-home address, do solemnly and sincerely declare that :
I have a birth certificate numbered certificate number, issued by the lovelovelovexx, on behalf of Johanes.
I have a pbuttport numbered pbuttport number, issued by lovelovelovelove, on behalf of Johanes.
I have degree certificate numbered certificate number, issued by lovelovelovexx, on behalf of Johanes Rudi Siswanto.
I have an academic transcript numbered certificate number, issued by lovelovelovelove, on behalf of Johanes Rudi Siswanto.
And I make this solemn declaration, as to the aforesaid, according to the laws in this behalf made, and subject to the punishment by law provided for any willfully false statement in any such declarion.
