What an intriguing document you have managed to unearth! For a start, I think there are Freedom of Information Act issues involved when DIMA tells Agents things that it fails to tell the public as well.
Secondly, you can obtain Forms 26 & 160 from here:
Scroll to the bottom of the web-page, click on the Health link and you will find both of the forms.
Thirdly, in the last couple of days I have become aware of a new practice by the Perth Offshore Parents Centre (POPC.) Seemingly, they are taking the photo from the application form, scanning it into the computer, and then pasting the same photo onto the meds forms electronically before despatching the forms to the applicants and-or their Agents. Presumably the address to which the meds are to be sent is also pre-entered on the form.
I shouldn't think the Panel Doctors will be enchanted by the allegation that they are the ones making the chicken-ups. The Panel Doctor involved with a friend of mine recently did not screw up. The LCU did that all by themselves, by sending my friend's CAT B meds to storage instead of to HSA-HAS.
