I agree with you 100% technically, but I simply do not believe that Jayne can reasonably be blamed for what has happened. She plainly got hoodwinked by a drongo who - if a member of your professional body (which seems unlikely to me) - should be struck off. Realistically, even though I am nothing of the sort, I could shove up a sign saying, "Gill Palmer, Migration Agent Extraordinaire, No 5467531" and it is highly unlikely that the average client would investigate my credentials. It is simply not what ordinary folks do, which is how bent solicitors and fake doctors get away with it too. The Courts know that, for sure, and I doubt DIMIA are far behind when it comes to twigging!
It would appear, however, that the real issue here is that the 16 year old boy is miserable with his Dad and new 'famille' plus Jayne has discovered that rescuing and protecting her beloved son might not be easy as she was encouraged to believe it would be at the outset.

1. Is there a way to rectify an application after it has been made and-or the visa granted on the basis of the false (though not intentionally so) statements?
2. If not, what is the solution? Tell DIMIA that Jayne isn't comfortable with their decision because she feels they were hoodwinked into it, so can she please bite the financial bullet and start again, this time on the basis of the real truth? It seems to me that as long as Jayne does not try to validate or activate the essentially fake visa, she should be allowed a second attempt, surely?
Reading your post, your own instinct seems to be, "Don't let let the situation get any worse," with which I entirely agree. Straightforward common-sense. This is not the time for alarmist nonsense, but for some clear-headed thinking about how to get Jayne out of this mess.
As to what the practical solutions might be, you are the pro in this field and Jayne could do a lot worse than to Instruct you formally, in my view.
I am relieved that you have decided to take an interest in this one.
