I emailed DIMA a while ago and asked them a similar questions so I'm going to post their response at the end of this message. We are applying for a send 136 visa and I am currently 6 months pregnant. From everything I've read and heard from DIMA if you have the baby before your application is finalized you can add the baby to your visa. If you get your visa first and then have the baby in your original country then you would need to apply for a separate visa for the baby, which I think is around $1000. So the key is you either get the visa early enough and have the baby in Oz, or you make sure that the baby is added before your application is complete. In my case I can't get the medicals done until after the baby is born, so there is no way the visa can be granted. So I will be doing the medicals for all of us and adding the baby at the same time.
To be sure you may need to call them to check for the requirements specific to your visa.

If a member of your family becomes pregnant while your application is being processed, you should notify the Adelaide Send Processing Centre as soon as possible.
It is a requirement for the send visas that all applicants can meet the health requirement for migration to Australia. If you or your spouse is pregnant, once the application reaches this stage, your application will be placed on hold until after the baby is born.

When the baby is born, you must provide a birth certificate, four pbuttport photos of the baby and notification of your change in circumstances. Your application will then proceed.
