108 Endocrinologist's report (based upon a current buttessment) required regarding diabetes, and hypertension, please. The report is to address history (including any previous hospitalisation), physical examination findings, state of target organs, management needs and prognosis.
Please include results of the following: resting ECG, glycosylated haemoglobin level, serum creatinine level, Urinary albumin-creatinine ratio (with appropriate predicted values), or 24-hour collection for urinary protein and creatinine clearance.
The report must be sealed by you, the Doctor, stamped & signed then sent from your clinic to the Local Clearance Unit at the following address:

I cant help you regarding the smoking as my partner managed to stop smoking nearly a year ago (in anticipation of life changes) and this was noted by his specialist in his report.
