On the form there is a section where you can request a check from Scottish Criminal Records Office and also from New Scotland Yard. I got both just to be on the safe side and it only cost me å£10 in total for both. Hope this helps. (I buttumed you were living in Scotland since you mentioned disclosure Scotland in your post!)
