If DIMIA ever finds out their first response will be to consider cancelling your visa, and if you become a citizen, they'll consider cancelling that too. Sincerely hope you're not identifiable through your location and other posts. Best you can do is tell the truth as soon as possible and go through the business of explaining why it would be unreasonable to ask for him to do a medical. A lot of other poor sods here have had to do that, what you've done won't be popular.

We had a case some time ago where a client had two children, one with his wife and one with a girlfriend, conceived a month apart. Ten years after he migrated to Australia the girlfriend in the home country suffered a very serious health problem and the illegitimate son needed to go to live with his father. Naturally he had never told his wife about the child and therefore had never included him in any relevant visa or citizenship application. He finally told his wife when the situation arose and she not only forgave him but insisted the boy come to live with them. It turned out that she "had known in her heart" about this other relationship and had been forgiving him all that time. She certainly loved him and wrote a particularly moving statement about her dawning awareness of the situation, and acceptance of her husband's frailty, and her readiness to embrace this new son. But DIMIA thoroughly considered cancelling his citizenship (and his wife's and legitimate child's citizenship) despite the obviously genuine nature of the documentation provided from the father, his wife, the son and the natural mother. When they'd finished considering how to punish him they decided, against every shred of evidence, however clearly with punitive intent, that he'd need to do a DNA test to prove he was the father. Of course, the test was positive and after a long battle DIMIA relented.
That story had a happy ending, but without an agent it would have ended in tears.
