With any ko competition luck plays an enormous factor. Hence sides like Barca have only won it twice. A Side like Real Madrid went for decades without winning it. United, during their dominance of the EPL sometimes went out because we weren't good enough against a certain team on the night, sometimes because of bad luck and poor refereeing decisions.
The point is that one slice of bad luck, or a bad decision and you're out. The best team doesn't always win. In the EPL there are 38 games in which the luck and decisions even themselves out and the best team does 'always' win.
Hence LFC were 'lucky' enough to win the CL last season, but only just about 'good' enough to challenge for 4th place in the league.
